Chronic care management is challenging. Care managers are expected to manage not just the clinical considerations of care, but also the behavioral and financial elements that drive positive outcomes with only 17-minutes per patient encounter. Not surpringly, a study found that less than 1 out of 5 recommendations made to patients are evidenced based and explains why year after year we keep spending more on supporting patients with chronic conditions. Today’s efforts clearly are not bending the cost curve.
Who are your first pilot customers? (It’s okay if you don’t have one yet, but if you do, please list): 
Our first customers have been independent clinics in California and we are in late-stage, active sales cycles with multiple health systems. Usage of our platform has resulted in exciting early results. We have helped our partners achieve 80% patient engagement at 6-months which has led to a 12mmHg reduction in systolic blood pressure for patients with hypertension and 93% of customers rating myNurse as very good/excellent.
What technologies do you use?: 
We have developed an NLP-based voice technology that provides clinical insights in real-time to care management teams supporting patients with chronic conditions between clinic visits over phone and video. The platform learns the words, phrases, and techniques top-performing care managers use to drive positive outcomes. It then shares this knowledge across the care management team with live assistance at the point of care. Meanwhile, the platform's workflow optimization automates administrative tasks, allowing clinicians to spend more time with patients, where they can go beyond addressing acute biometric clinical needs.
